Non communicable disease clinic

Non communicable disease care for diabetes, hypertension and other chronic diseases. The NCD unit runs clinics in the rural (CHTC Mugalur) and one at the UHTC Austin Town.  The clinic based services offered include consultation, anthropometry, blood sugar and blood pressure measurement and advice on medication management and lifestyle changes, ensuring  medication adherence and regular follow up. Electronic patient records are maintained at the clinic and the patient is given a comprehensive NCD file. Colour coded diabetes monitoring charts are used to track glycemic control status of the patients. Patient data is entered in an excel sheet at the end of each clinic. Counselling for diabetic foot and lifestyle changes is a part of the program. Control rates for diabetes and hypertension are calculated every month and monitored as outcome indicators. The population level services include follow up of patients at household level by community health workers. During these household visits, the community health workers reinforce advice on lifestyle changes, identify barriers to following these lifestyle changes, screen family members of patients for hypertension and diabetes and remind the patients to keep clinic appointments. 

The clinic has an additional service which includes a Diabetic Foot Care clinic, where in a doctor from the PMR department visits the facility on NCD clinic days and offers Diabetic foot care services. MCR footwear is available for patients after foot measurement by staff from foot care division at a low cost.
