ICMR - Improving Glycemic Control
Project Description
Aim: To develop a health care delivery model for diabetes that will improve glycemic control and quality of care for Diabetes Mellitus in urban underprivileged settings
Objectives:
To design and identify the most promising delivery model to address barriers to glycemic control through formative research and iterative quality improvement (QI) approach.
To implement and demonstrate feasibility of the most promising model that will improve glycemic control rates by 20% and evaluate quality of care.
