Efficacy of Ultrasound-guided Transversus Abdominis Plane Block Compared to Standard Analgesic Protocol for Postoperative Pain Management Following Elective Cesarean Section: A Prospective Comparative Study

Background: The transversus abdominis plane (TAP) block has gained prominence as a key component of multimodal analgesia in the context
of lower abdominal surgical procedures.
Aims and objectives: The main aim of our study was to evaluate the effectiveness of the ultrasound-guided TAP block in extending the duration
of analgesia. Additionally, we sought to determine the total quantity of opioid doses administered over a 24-hour period and to monitor the
occurrence of various opioid-induced side effects.
Materials and methods: A prospective comparative study was conducted involving the American Society of Anesthesiologists (ASA) I and II
patients enduring cesarean delivery. The study included 30 patients in each of the two groups. Patients in group I were administered a standard
analgesic regimen consisting of 50 mg of intramuscular meperidine upon request and 1 gm of intravenous acetaminophen every 8 hours. In
contrast, group II received an ultrasound-steered TAP block utilizing 0.25% bupivacaine in addition to the standard analgesic protocol.
Results: In group I, the average duration until the initial administration of meperidine was 196 minutes, in contrast to 363 minutes (p < 0.001)
observed in group II. Over a 24-hour period, the total consumption of meperidine in group I amounted to 201 mg, compared to 103 mg in
group II. Additionally, group II exhibited superior outcomes in terms of visual analog scale (VAS) scores, incidence of postoperative nausea and
vomiting (PONV), and maternal satisfaction.
Conclusion: The findings of the study indicate that the administration of a ultrasound-guided TAP block following a cesarean section significantly
diminishes the need for postoperative analgesics. Therefore, it is advisable to incorporate this technique into a multimodal analgesic approach.
